What a medical document connects

The official English PT33 form links the prescriber's identity and licence, the patient's identity, the disease or condition considered, and the quantity and period prescribed. This makes the document traceable. A screenshot with only a patient name or a promise in chat does not provide the same chain of information. Check these fields in the official English PT33 form.

A practical decision map

1. Confirm current requirements in an official source.
2. Speak with a qualified professional if medical assessment is needed.
3. Check that the document matches the patient and prescriber.
4. Ask the seller what must be presented and retained.
5. Only then confirm stock, pickup or other service details.

This order separates a clinical decision from a commercial request and reduces the risk of sending sensitive data too early.

Tourists and residents

The official PT33 form includes nationality and an ID or passport number. A visitor may therefore need a passport-based identity check, while a resident may use another accepted identifier. These fields do not guarantee eligibility: the licensed professional and current official process determine whether the document can be issued in a particular case.

What the document does not do

A medical document does not turn product descriptions into treatment advice, guarantee that a strain is suitable, reserve stock, remove age or location rules, authorise public smoking, or settle questions about travel and transport. Each question needs its own current answer.

How to verify information

Use the DTAM controlled-herb portal for current forms and notices, read the official form rather than a copied template, and contact the DTAM Controlled Herb Service Center when a rule or field is unclear. On a store website, look for a named author, a recent source-check date, clear limits and links to primary sources.
