What PT33 is — and what it is not

PT33 is the controlled-herb prescription form published through Thailand's DTAM controlled-herb portal. It records a prescriber's decision for an identified patient, a stated disease or condition, a daily amount and a defined period. It is not a store membership card, a blanket travel permit or a guarantee that a prescription will be issued.

Part 1: check the prescriber block

The official English PT33 form asks for the prescriber's name, licensed professional category, professional licence number and address. Do not treat an unsigned template or a document with no licence details as complete. Ask the issuing professional or official service when a field is unclear.

Part 2: match the patient identity

The patient block on the official form contains name, age, nationality and an ID-card or passport number. Check spelling and every character against the identity document. A nickname, shortened name or transposed passport digit can break the connection between the person and the form.

Part 3: read the medical and quantity fields

The form includes the disease or condition, daily dosage in grams, number of days and total quantity. The values should be internally consistent and come from the prescriber's decision. Store staff and website copy should not invent, change or interpret a dosage for the patient.

The 30-day note

The English PT33 form states that a prescription must not exceed 30 days per issuance. This is a limit printed on the official form, not a promise that every patient will receive 30 days. The actual period and quantity depend on the qualified prescriber's assessment and current rules.

Part 4: support and signatures

The form says the diagnosis should align with relevant professional guidance and supporting documents. It also provides places for prescriber and patient names and signatures. Check required signature areas and return any correction to the issuing professional.

• Keep the identity document used for the patient fields available.
• Write the legal name exactly as it appears on that document.
• Ask who conducts the clinical review and which licence applies.
• Ask how the completed form will be delivered and verified.

Pause when the legal name differs from the passport, the licence number is missing, daily amount and total quantity do not match, the issue period is unclear, a signature is absent, or someone promises approval without assessment. Record retention and privacy

The official English form says the entrepreneur shall retain the prescription for one year. Before sharing identity documents, ask which entity keeps the record, what format is stored, who can access it and how additional files such as passport copies are handled.
